Onboard Incident Investigation
Marine incidents require a timely, structured and independent investigation to establish what happened, understand why it happened and prevent recurrence.
We conduct onboard incident investigations on behalf of shipowners, managers, operators and other maritime stakeholders. Our investigators attend the vessel to examine the incident scene, secure relevant evidence and evaluate the effectiveness of existing operational and management controls.
The investigation may include interviews with shipboard and shore-based personnel, review of logbooks, permits, procedures, maintenance records, voyage data, photographs, equipment condition and other available evidence. Particular attention is given to operational decision-making, human factors, supervision, training, communication, risk assessment and compliance with company procedures and applicable regulations.
Our approach focuses on identifying immediate, underlying and root causes rather than assigning blame. Findings are supported by objective evidence and presented in a clear, professional investigation report.
Depending on the scope, the report may include a sequence of events, causal analysis, control failures, contributing factors and practical corrective and preventive actions.
