INVESTIGATIONS

Remote Incident Investigation Services

When onboard attendance is not immediately practical, our Remote Incident Investigation Services provide shipowners, managers and operators with a structured, timely and evidence-based investigation solution.

Each investigation is conducted using a specifically designed methodology based on the nature, severity and circumstances of the incident. Our investigators coordinate with vessel and shore management to collect and review relevant evidence, including logbooks, photographs, videos, permits, procedures, maintenance records, electronic data and witness statements.

Where available, the investigation includes detailed analysis of Voyage Data Recorder (VDR) information, such as bridge audio, radar images, vessel position, course, speed, alarms, communications and navigational equipment data. This supports accurate reconstruction of the sequence of events and evaluation of operational decision-making.

Remote interviews are conducted with relevant shipboard and shore-based personnel to assess human factors, supervision, competence, communication, procedural compliance and management-system controls.

The final report presents the established facts, timeline, immediate and underlying causes, contributing factors and practical corrective and preventive recommendations. Any evidence limitations or areas requiring onboard verification are clearly identified.
